limited to counseling, therapy sessions, wellness consultations, and related healthcare services. All

services are provided by licensed professionals in accordance with applicable state and federal regu

3. Appointment Scheduling and Cancellations

3.1 Scheduling

Appointments may be scheduled by phone at (815) 782-4196, email at office@mytherapywell

ness.com, or through our online portal when available.

3.2 Cancellation Policy

• Appointments must be cancelled at least 24 hours in advance

• Late cancellations or no-shows may result in a cancellation fee

• Emergency cancellations will be handled on a case-by-case basis

3.3 Rescheduling

We will make reasonable efforts to accommodate rescheduling requests when provided with ade

quate notice.

4. Payment Terms

4.1 Fees

All fees for services are due at the time of service unless other arrangements have been made in ad

vance.

4.2 Insurance

We accept various insurance plans. Patients are responsible for verifying coverage and understanding

their benefits, copayments, and deductibles.4.3 Outstanding Balances

Unpaid balances may result in suspension of services and may be referred to collection agencies.

5. Patient Responsibilities

5.1 Accurate Information

You agree to provide accurate and complete information regarding your health history, insurance,

and contact details.

5.2 Treatment Compliance

You agree to follow treatment recommendations and communicate any concerns or changes in your

condition.

5.3 Respectful Conduct

You agree to treat all staff and other patients with respect and courtesy.

6. Confidentiality and Privacy

Your privacy and confidentiality are protected under HIPAA and state privacy laws. Please refer to our

formation.

7. Limitations of Liability

7.1 Professional Services

Our services are provided by licensed healthcare professionals. However, we cannot guarantee specif

ic outcomes or results from treatment.

7.2 Emergency Situations

Our services are not intended for emergency situations. In case of a mental health emergency, please

call 911 or go to the nearest emergency room.

7.3 Limitation of Damages

To the fullest extent permitted by law, Advanced Therapy & Wellness Center shall not be liable for

any indirect, incidental, special, or consequential damages arising from the use of our services.

7.4 No Medical Advice; Informational Content

The content on our website, patient portal, emails, text messages, social media, educational materials,

and other non-clinical communications is provided for general informational purposes only and is not

a substitute for professional medical advice, diagnosis, or treatment. Do not disregard, avoid, or delay

obtaining medical or mental health advice from a qualified professional because of something you

have read on our website or received from us in non-clinical communications.7.5 Use at Your Own Risk

content provided by us (collectively, "Online Services") is at your own risk. You are solely responsible

email, or text message for appointment reminders, treatment updates, and administrative purposes.9. Minors

Services provided to minors require parental or guardian consent. Parents/guardians are responsible

for ensuring minors comply with these terms and treatment recommendations.

10. Intellectual Property

All content, materials, and information provided during treatment sessions or on our website are the

intellectual property of Advanced Therapy & Wellness Center and may not be reproduced or distrib

uted without written consent.

11. Termination

11.1 Patient-Initiated

You may terminate services at any time with written notice. You remain responsible for any outstand

ing fees.

11.2 Provider-Initiated

We reserve the right to terminate services for non-compliance with treatment recommendations,

non-payment, disruptive behavior, or other reasons deemed appropriate by our clinical staff.

12. Modifications to Terms

We reserve the right to modify these Terms of Use at any time. Changes will be effective immediately

upon posting. Continued use of our services constitutes acceptance of any modifications.

13. Governing Law

These Terms of Use shall be governed by and construed in accordance with the laws of the State of

Illinois, without regard to its conflict of law provisions.

My Therapy and Wellness SMS Program
The My Therapy and Wellness SMS Program provides appointment reminders, scheduling confirmations, wellness-related updates, care coordination messages, and occasional administrative or service-related notifications to individuals who opt in to receive text messages from our practice.
You can cancel the SMS service at any time. Simply text "STOP" to the shortcode. Upon sending "STOP," we will confirm your unsubscribe status via SMS. Following this confirmation, you will no longer receive SMS messages from us. To rejoin, sign up as you did initially, and we will resume sending SMS messages to you.
